If you've received services from SASSMM, we invite you to participate in the a voluntary, anonymous survey. The Adult Client Questionnaire and Empowerment and Satisfaction Survey (ACQES) is a survey that provides feedback to us on how we did in providing supportive services to you to address your goals around seeking healing, safety and justice, as it relates to your experience with sexual violence. You submitting this survey will not have an impact in how you receive services from SASSMM, ACQES Survey .
