For general questions, comments, or feedback, please fill out our online contact form. You may choose to remain anonymous. If you'd like to file a grievance, you may also use our online Grievance Form.

If you've received services from SASSMM, we invite you to participate in a voluntary, anonymous survey. The Survivor Voice: Experience Survey is a survey that provides feedback to us on how we did in providing supportive services to you to address your goals around seeking healing, safety and justice, as it relates to your experience with sexual violence. Submitting this survey will not have an impact in how you receive services from SASSMM. If you have questions about how to access and submit the survey form, please feel free to call the Helpline, and an advocate can speak with you to address those questions: 1-800-871-7741

Click here to take the survey.

*Please note: These forms are intended for general contact and feedback purposes only. Email is not a safe or confidential way to talk about sexual violence. If you'd like to speak with an advocate, we can privately contact you to provide support, information, and resources. Just let us know!
