For general questions, comments, or feedback, please fill out our online contact form. You may choose to remain anonymous. If you'd like to file a grievance, you may also use our online Grievance Form.

If you've received services from SASSMM, we invite you to participate in the a voluntary, anonymous survey. The Survivor Voice: Experience Survey is a survey that provides feedback to us on how we did in providing supportive services to you to address your goals around seeking healing, safety and justice, as it relates to your experience with sexual violence. You submitting this survey will not have an impact in how you receive services from SASSMM. To access this form, click on the following link below. Once you access the MECASA website, click on the tab that reads, "Client Assessment Tools". Once you click on the tab, you'll see a drop down list of the survey in PDF format. The survey is available to take in the following languages: English, Arabic, French, Portuguese, Somali, and Spanish. If you have questions about how to access and submit the survey form, please feel free to call the Helpline, and an advocate can speak with you to address those questions: 1-800-871-7741
